Home from hospital support

From physiotherapy and occupational therapy, through to personal care and cleaning — having extra help when you return home from a hospital stay can boost your recovery and help you settle back in.

Though you may be well enough to leave hospital, returning home may require a period of support to help you regain your independence and confidence. Practical help with everyday personal tasks like bathing and dressing is available and can be a lifeline while you recover. This could be for a limited time, or be part of an ongoing package of care if your health condition is likely to be longer term.

Support with home services such as cleaning, gardening and shopping can reduce the pressure on you to recover and allow you to take as much time as your body needs.

Primary Homecare support from Cardinal Healthcare is available for as little or as long as you need it and starts from just one hour per week, through to full-time live-in care.

Reablement services

Sometimes more intensive home help may be needed following an unexpected health event or hospital stay. Reablement services may be recommended by the hospital as part of your care plan.

Reablement services can help you to settle back into home, re-establish routines, re-learn tasks and adjust to any support aids recommended for you by the hospital. If your support needs will be ongoing, reablement services can help you learn new ways of doing things around the home, such as washing, moving around and preparing meals.

Like Primary Homecare, the aim of reablement (sometimes called enablement) services is to help you to live as independently as possible while remaining in your own home. Reablement services are usually provided for a fixed period. After this time, your progress and support needs will be reviewed to determine if longer-term or ongoing support is needed.

Support for carers

If the person you care for is returning home after a stay in hospital, it’s important that you have enough support to make sure your own needs are met as well as theirs. Having a period of home or personal care support can help you re-adjust to having them back in the home and make any necessary adjustments to your routines.

If their support needs have increased since they were last at home, having Primary Homecare support for a few weeks will help to identify whether you’ll be able to continue caring for them and manage your own needs.

You may find that additional ongoing support at home will be needed to help you both remain at home. If that’s the case, Cardinal Healthcare can provide a flexible package of support that can be reviewed as often as necessary and last for as long as you need it.

Respite care

Cardinal Healthcare can provide occasional, through to regular day and respite care at its Baylham and Barham Care Homes in Suffolk. Respite care can be a lifeline for carers, providing valuable ‘time out’ from their caring responsibilities and quality time with other family members.

Carers can be confident that the person they care for is safe and well looked after by people they can trust. Regular weekends can also be arranged.
